Diagnosis

• First-line: ERCP (for duct-to-duct anastomotic strictures)
• Second-line: PTBD (percutaneous transhepatic biliary drainage)
• Hepaticojejunostomy (HJ) strictures: PTBD preferred; policy of maintaining PTBD for one year rather than multiple ERCP sessions
• Protocol: Upsizing stents every 2–3 months; stricture remodeling confirmed when inflated balloon passes freely with free dye drainage
• IV antibiotics mandatory for 48 hours (immunosuppressed patients are at high risk for life-threatening cholangitis)
