Classification of Cholangiocarcinoma

• Intrahepatic CCA: ~5–10% of cases; presents more often with abdominal pain than jaundice
• Perihilar CCA (Klatskin tumor): >50% of diagnosed cases; most relevant to transplant discussion
• Distal CCA: Typically managed with pancreaticoduodenectomy (Whipple procedure); generally outside the scope of transplantation

Risk Factors

• Primary sclerosing cholangitis (PSC): 8–20% lifetime incidence of CCA
• Choledochal cysts: ~24% lifetime risk, increasing with duration
• Chronic hepatitis B/C, ulcerative colitis (even without PSC), metabolic/steatotic liver disease

Diagnostic Workup — Key Recommendations

• MRI/MRCP is the preferred initial imaging — obtain *before* any biliary instrumentation when CCA is suspected
• Use Bismuth-Corlette classification to define ductal extent; Bismuth IV (involvement of second-order radicals) = typically surgically unresectable but may be a transplant candidate
• Assess vascular involvement on cross-sectional imaging
• CA 19-9 is the primary serum biomarker — most meaningful *after* biliary decompression, as obstruction alone can elevate levels; markedly elevated CA 19-9 in a decompressed system suggests distant/unresectable disease
• Obtain AFP if mass lesion present (to evaluate for mixed HCC/CCA)
• ERCP preferred over PTC to avoid perforation of the tumor plane
• Endoscopic ultrasound (EUS): Useful for regional lymph node sampling and assessment of radial tumor margin
• Staging: Chest CT and PET scan recommended; PET has poor sensitivity for lymph node metastases (~12%) but good sensitivity for ruling out distant metastases

Critical Clinical Pearls

• ⚠️ Do NOT perform percutaneous biopsy — high risk of peritoneal tumor seeding, which precludes surgical resection or transplantation
• ⚠️ A negative biliary brushing does NOT exclude malignancy — diagnosis often requires multiple procedures
• Involve surgical consultation early to preserve all treatment options
• Prefer ERCP for biliary decompression; segmental decompression can alleviate jaundice even if only one ductal system is accessed
