Coagulation in Liver Disease

• Liver disease produces a rebalanced but fragile hemostatic state, affecting both pro- and anti-thrombotic pathways simultaneously
• The "Tripodi model" (2011) describes hemostasis in liver disease as a delicate balance — patients are vulnerable to tipping toward either bleeding or thrombosis under physiologic stress (e.g., infection, decompensation)
• Traditional coagulation markers (INR, platelet count) are unreliable for assessing true bleeding risk in cirrhosis
• Viscoelastic testing (TEG/ROTEM) is increasingly used to guide resuscitation; literature is still evolving outside the perioperative setting

Management of Coagulopathy — Key Recommendations

• Avoid over-transfusion of pRBCs or FFP in acute bleeding; raises portal pressure and worsens variceal hemorrhage risk
• Routine platelet transfusion for variceal hemorrhage or standard bedside procedures is not recommended
• Vitamin K is not routinely beneficial unless vitamin K antagonist-induced coagulopathy or deficiency is suspected (e.g., cholestasis, antibiotic use)
• Alternatives to standard biopsy: transjugular liver biopsy to reduce procedural bleeding risk
• Adjunctive hemostatic agents: intranasal DDAVP, aminocaproic acid (e.g., dental extractions)
• Factor VIIa and 4-factor PCC: limited evidence; not extensively validated in most clinical scenarios
• Anti-fibrinolytic agents (aminocaproic acid, tranexamic acid): emerging utility for non-portal pressure–related bleeding

Portal Vein Thrombosis — Diagnosis and Classification

• 2021 AASLD guidelines recommend a standardized nomenclature system for documenting PVT characteristics and treatment response
• Doppler ultrasound is the preferred first-line imaging modality (high sensitivity/specificity, widely available), though limited by bowel gas, obesity, and SMV visualization
• Contrast CT or MRI are interchangeable for confirmation and extent assessment; choice depends on availability and patient factors
• Endoscopic ultrasound is emerging but has lower sensitivity/specificity and is not routine
• Rule out malignant thrombus in all new PVT — up to 20% of HCC patients develop concurrent PVT
• AVENA criteria (modality-agnostic): ≥3 of the following suggest malignant thrombosis — AFP >1000, portal vein expansion, arterially enhancing thrombus, neovascularity, thrombus contiguous with HCC
• Cavernous transformation may develop within 6–20 days, though more commonly takes longer, especially in cirrhosis
