Porto-Pulmonary Hypertension (PoPH)

Definition & Classification:

• PoPH falls under WHO Group 1 Pulmonary Arterial Hypertension (pre-capillary, not post-capillary)
• Characterized by vasoconstriction, thrombosis, and pulmonary vascular remodeling
• Distinguished from pulmonary venous hypertension (Group 2)

Updated Diagnostic Criteria (post-2023):

• Mean pulmonary artery pressure (mPAP) > 20 mmHg
• Pulmonary artery wedge pressure (PAWP) < 15 mmHg
• Pulmonary vascular resistance (PVR) > 2 Wood units (160 dynes)
• Presence of portal hypertension (confirmed clinically or via hepatic venous pressure gradient)

Key Hemodynamic Relationships:

• PVR = (mPAP − PAWP) ÷ Cardiac Output
• mPAP = (PVR × CO) + PAWP
• Diastolic pulmonary gradient = Diastolic PAP − PAWP

Pathophysiology:

• Portal hypertension allows vasoactive substances (notably endothelin-1) to bypass hepatic clearance
• Leads to endothelial dysfunction, sustained pulmonary arterial vasoconstriction
• Chronic endothelial injury drives medial hypertrophy, intimal fibrosis, and smooth muscle proliferation
• Occurs in a background of hyperdynamic, high-flow circulatory state

Echocardiographic Screening:

• Tricuspid regurgitant velocity (TRV) > 2.8 m/s is a key screening threshold
• Echo parameters (ventricular morphology, PA diameter, IVC, right atrium) correlate well with invasive measures:

- AUC 0.936 for mPAP > 20 mmHg; AUC 0.933 for mPAP > 25 mmHg

• Limitations: TRV jet may be undetectable in patients with obesity, ascites, or lung disease → suboptimal acoustic windows
• If TRV cannot be estimated, proceed to right heart catheterization (RHC) if clinical suspicion is high

Differential Diagnosis — Hyperdynamic Circulatory State:

• Cirrhotic patients have high cardiac output due to splanchnic vasodilation and reduced effective arterial blood volume
• Hemodynamic pattern: ↑ mPAP, ↑ CO, low PVR, low-normal PAWP
• Must be distinguished from true PoPH (where PVR is elevated)
• Highlights importance of RHC over echocardiography alone for definitive diagnosis
