Diagnosis
- PBC diagnosis requires 2 of 3 criteria (per both AASLD and EASL guidelines):
- Chronic, unexplained alkaline phosphatase (ALP) elevation
- Positive antimitochondrial antibody (AMA)
- Compatible liver histology (bile duct injury, granulomas, lymphocytic cholangitis)
- Liver biopsy is not mandatory if 2 of 3 criteria are met
- AMA-negative cases (~5% of patients): consider anti-SP100 or anti-GP210 antibodies; a reticular ANA pattern carries equivalent diagnostic weight to AMA
- Exclude competing etiologies: obstruction (ultrasound), PSC (MRCP/EUS), chronic viral hepatitis, drug-induced liver injury
- Overlap with autoimmune hepatitis (AIH) should be considered when elevated ALT and elevated IgG are present alongside PBC criteria; liver biopsy is warranted in this setting
Disease Characteristics
- Predominantly affects middle-aged women; frequently asymptomatic at presentation
- Key symptoms: fatigue, pruritus, sicca syndrome (dry eyes/mouth), fat-soluble vitamin deficiency, cognitive impairment, osteoporosis
- Pre-sinusoidal portal hypertension can occur even at F3 fibrosis stage (splenomegaly, thrombocytopenia)
- Smoking accelerates disease progression; smoking cessation counseling is recommended
Risk Stratification
- Fibrosis assessment: transient elastography (FibroScan), 2D shear wave elastography, or MR elastography — note that cutoff values differ by modality

Standard of Care
- Ursodeoxycholic acid (UDCA): first-line therapy
- Dose: 13–15 mg/kg/day (once daily or divided BID)
- Generally well tolerated; ~20–30% of patients experience intolerance (reported side effects include hair loss, GI symptoms)
- Reduces mortality and need for liver transplantation, particularly when initiated at early disease stages; less effective in advanced disease
- Obeticholic acid (OCA): second FDA-approved agent (approved 2016); indicated for inadequate UDCA response
