Defining Refractory Ascites (AASLD Criteria)
- Ascites not mobilized or early recurrence not prevented despite maximal diuretic doses
- Diuretic-resistant: No response to spironolactone 400 mg/day + furosemide 160 mg/day
- Diuretic-intractable: Complications precluding adequate diuretic use, including:
- Creatinine increase ≥100% or to >2.0 mg/dL (AKI threshold)
- Hyponatremia: decrease >10 mEq/L or absolute value <125 mEq/L
- Hyperkalemia (with flexible limits)
- Hepatic encephalopathy triggered by hypovolemia
- Frequently recurring ascites: ≥3 episodes within 12 months despite adequate diuretics (Baveno consensus definition)
Optimizing Medical Therapy Before Declaring Refractory Ascites
- Avoid premature abandonment of diuretics — many patients classified as refractory may still respond with medication review
- Medications to discontinue or avoid:
- NSAIDs (universally contraindicated in ascites; celecoxib studied in small, short-term study only)
- ACE inhibitors and angiotensin receptor blockers (ARBs) — cause significant GFR reduction and arterial pressure decrease in Child B/decompensated patients
- Calcium channel blockers and vasodilators
- SSRIs (contribute to hyponatremia, complicating diuretic therapy)
- Beta-blockers: Cause renal impairment only when they cause hypotension; carvedilol may benefit patients with concurrent cardiac dysfunction or ACLF (data from Indian study)
- Statins: Pharmacokinetics severely altered in Child B patients:
- Systemic exposure to atorvastatin increases 11-fold
- Simvastatin 20 mg associated with substantial rhabdomyolysis risk (LEOHOPE trial, EASL 2023)
- Rosuvastatin has minimally abnormal pharmacokinetics — preferred if statin required
- AKI from diuretics: Evidence suggests mild-to-moderate AKI episodes cause small incremental worsening of subsequent kidney function when baseline eGFR, proteinuria, and covariables are accounted for — clinicians should not overreact to minor creatinine rises

Albumin Infusion
