• Acute vs. chronic hyponatremia: Acute hyponatremia is defined as duration < 48 hours; the brain has insufficient time to adapt, increasing seizure and herniation risk; in chronic hyponatremia, astrocytes extrude potassium and organic osmolytes (e.g., myoinositol, glutamine) to mitigate cerebral edema
• Astrocyte dysfunction: Adaptive osmolyte extrusion protects against swelling but impairs astrocyte function and neurotransmitter release, potentially worsening hepatic encephalopathy
• Sodium supplementation ineffective: Oral sodium tablets do not correct dilutional hyponatremia and may worsen fluid overload; a common misconception in non-specialist settings
• MELD-Na: Serum sodium is incorporated into organ allocation scoring; every reduction from 140 to 125 mEq/L increases pretransplant mortality by approximately 5% per unit decrease
