Definition and Scope of Endohepatology

EUS-Guided Liver Biopsy — Patient Selection
Indications:
- Patients already requiring upper GI endoscopy or EUS (combined procedure increases cost-effectiveness)
- Anxious patients with poor breath-holding ability (sedation can be administered)
- Obese patients
- Patients with mild ascites
- Cases requiring bilobar sampling (e.g., fatty liver disease, pre-transplant evaluation)
- Patients undergoing EUS for fundal varices (coil/glue therapy can be combined)
Contraindications:
- Platelet count < 50,000
- Coagulopathy / INR > 1.5–2.0
- Significant ascites
Technical Recommendations for EUS-Guided Liver Biopsy
- Preferred needle: 19G FNB needle with Franseen (crown-tip) design — superior to forward bevel, reverse bevel, and Menghini-style needles
- Needle gauge hierarchy: 19G FNB > 22G FNB > 25G FNB
- Biopsy type: FNB needles superior to FNA needles
- Suction technique: Wet heparin suction (needle flushed with 1:1,000 diluted heparin saline, ~2 cc) superior to dry suction or no suction
- Technique: One pass with three actuations (fanning technique) superior to three separate passes
- Always apply Doppler prior to and during needle withdrawal to avoid vascular injury
- Blood patch technique: If Doppler signals persist after needle placement, hold needle for 30 seconds; inject small amount of material into tract upon withdrawal to tamponade bleeding risk
- Tissue handling: Flush with saline (not stylet reinsertion) to avoid tissue fragmentation; macrodissection in petri dish; fix in 10% formalin
- Communicate with pathologist regarding 19G vs. conventional 16G core differences
