Cardiovascular Disease Burden Post-Transplant:

Pathophysiology — Five Inflammatory Pathways:
1. Hepatocyte injury → DAMPs (Damage-Associated Molecular Patterns) → systemic inflammation
2. Portal hypertension → splanchnic vasodilation → gut microbiome translocation → PAMPs → inflammation
3. TNF-α pathway — direct elevation or indirect activation via cannabinoid-1 receptor
4. Angiotensin II — elevated in cirrhosis → inflammation and oxidative stress
5. Interleukin pathway — dysregulation promoting chronic systemic inflammation
These pathways collectively lead to: myocardial inflammation → oxidative stress → myocardial fibrosis → cardiac remodeling → dysfunction
Diagnostic Criteria (2005 World Congress of Gastroenterology):
- *Systolic dysfunction*: EF <55% or blunted response to stress
- *Diastolic dysfunction*: E/A ratio <1, IVRT >80 ms, deceleration time >120 ms
Limitations of Current Diagnostic Criteria:
- EF: Artificially elevated due to reduced afterload from systemic vasodilation — unreliable in cirrhosis
- Stress testing: Confounded by beta-blocker use (some with half-lives up to 3 days); no universal definition of "blunted response"
- E/A ratio: Preload-dependent; changes acutely (demonstrated to shift within 20 minutes post-paracentesis); age-dependent; subject to pseudonormalization (normal and Grade II diastolic dysfunction visually indistinguishable)
- IVRT: Exhibits a U-shaped phenomenon — shortens as diastolic dysfunction advances, making interpretation unreliable across disease stages
