Definitions of ACLF — Three Major Consortia

European (EASL-CLIF Consortium) — CANONIC Study:

• Based on prospective data from 1,343 patients (2011)
• Considers 6 organ systems: liver, kidney, brain, coagulation, circulation, respiration
• Diagnostic cut-offs are evidence-based (derived from hazard ratios for 28-day mortality), not expert opinion
• ACLF graded 1–3 based on number of organ failures
• Applies to patients non-electively hospitalized with acutely decompensated cirrhosis (AD-CLD)
• Considered the most comprehensive and broadly applicable definition

North American (NACSELD) Consortium:

• Based on 2,675 patients; focuses on ICU patients
• Considers only 4 organ systems: kidney, brain, circulation, respiration
• Does not include liver or coagulation failure
• ACLF defined as ≥2 organ failures
• Primarily applicable to severely ill, ICU-level patients

Asian (APASL) Consortium:

• Based on expert opinion and less transparent data collection methodology
• Applies to compensated or non-cirrhotic chronic liver disease
• Requires: acute hepatic insult (bilirubin ≥5 mg/dL + INR ≥1.5) complicated within 4 weeks by ascites and/or encephalopathy
• Excludes extrahepatic precipitants and non-hepatic organ failures (kidney, respiratory, circulatory)
• May represent an earlier stage of the ACLF trajectory

Reconciling Definitions

• A proposed hypothesis (published in *Journal of Hepatology*): APASL captures early-stage, EASL-CLIF intermediate-stage, and NACSELD late-stage disease along a continuous trajectory
• Confirmation requires large, international, prospective longitudinal studies

Management Principles

• Diagnose and treat precipitants urgently (especially infection and alcohol-related hepatitis)
• Provide organ-supportive care in ICU or intermediate care unit
• Goal of medical therapy: bridge to liver transplantation
• Empiric broad-spectrum antibiotics recommended for infection-triggered ACLF
• Antibiotic adequacy is critical — inadequate empiric therapy is independently associated with significantly higher 28-day mortality
• Multi-drug resistant organisms represent a major challenge to empiric antibiotic strategies
