Core Pharmacologic Principle

• Tacrolimus, cyclosporine, sirolimus, and everolimus are all metabolized via CYP3A4 and transported by P-glycoprotein
• CYP3A4 inhibitors → ↑ immunosuppression levels → toxicity risk
• CYP3A4 inducers → ↓ immunosuppression levels → rejection risk

CYP3A4 Inhibitors (Increase CNI/mTOR Levels)

CYP3A4 Inducers (Decrease CNI/mTOR Levels)

• Rifampin: Strongest inducer; dramatically reduces tacrolimus levels
• Anti-epileptics: Phenytoin, carbamazepine, phenobarbital

- ⚠️ Levetiracetam has minimal CYP interaction → safer alternative in transplant patients

• St. John's Wort (herbal supplement): Significant inducer; clinically important and frequently tested

Azathioprine + Allopurinol Interaction

• Azathioprine → converted to 6-mercaptopurine (6-MP) → normally broken down by xanthine oxidase
• Allopurinol inhibits xanthine oxidase → massive accumulation of toxic metabolites
• Result: Severe bone marrow suppression (profound leukopenia, pancytopenia)
• Board recommendation: This combination should generally be avoided; if unavoidable, requires major dose reduction and intensive monitoring

Mycophenolate Mofetil (MMF) Interactions

• MMF interactions primarily involve altered GI absorption, not CYP metabolism
• Antacids (magnesium- or aluminum-containing): Bind MMF in the gut → reduced absorption → lower drug levels
• Cholestyramine: Interrupts enterohepatic recirculation → reduces MMF exposure
