• Critical insight: Early graft loss typically presents with both patterns simultaneously

Primary Non-Function (PNF)

• Occurs classically within post-operative days 1–3
• Features: Massive AST elevations (frequently >5,000–10,000 and rising), rapidly worsening INR, persistent lactic acidosis, hemodynamic instability, minimal/absent bile production
• Must exclude HAT via Doppler before diagnosing PNF
• Confirmed PNF → urgent re-listing under Status 1A

Hepatic Artery Thrombosis (HAT)

• Classically presents around post-operative days 5–10
• Key pattern: Initial improvement followed by sudden enzyme spike (AST/ALT), fever, leukocytosis, rising bilirubin
• Requires immediate Doppler ultrasound (not biopsy, not delayed)
• If Doppler equivocal → escalate to CTA or MRA
• Early HAT: potentially salvageable via surgical revision or thrombectomy

Salvageable vs. Requires Re-Transplantation

Status 1A Listing (Board-Critical Concept)

• Indications include: acute liver failure, PNF after liver transplant, and HAT with graft failure (center-dependent criteria)
