Rebalanced Hemostasis

• Post-transplant patients are neither simply hypocoagulable nor hypercoagulable — they exist in a fragile, rebalanced hemostatic state
• Procoagulant factors decrease (clotting factors), but anticoagulant factors also decrease (Protein C, Protein S, Antithrombin III)
• Platelets are low, but von Willebrand factor (vWF) is elevated, providing partial compensation
• The system can tip toward bleeding or thrombosis with any physiologic stress (infection, surgery, shock, transfusion)

INR After Liver Transplant

• INR elevation is expected in the early post-transplant period due to the graft's synthetic recovery phase; Factor VII (shortest half-life) normalizes last
• INR does not equal bleeding risk — it measures only procoagulant pathways; it does not reflect anticoagulant factor reduction, platelet function, or vWF
• Elevated INR alone is NOT an indication for FFP transfusion in a hemodynamically stable, non-bleeding patient
• Risks of unnecessary FFP: volume overload, elevated portal pressures, transfusion-related acute lung injury (TRALI), and disruption of hemostatic balance

Indications for FFP Transfusion

• ✅ Active bleeding with suspected coagulopathic contribution
• ✅ Planned high-risk invasive procedure with markedly elevated INR (e.g., ERCP with sphincterotomy)
• ✅ Massive transfusion in setting of ongoing hemorrhage with dilutional coagulopathy
• ❌ Mild-to-moderate INR elevation (e.g., 1.8–2.2) in a stable, non-bleeding patient

Platelet Transfusion

• Thrombocytopenia is expected in the first 24–48 hours post-transplant (dilution, consumption, splenic sequestration, graft-related hemostasis)
• Indications for platelet transfusion:

- Active bleeding with thrombocytopenia as a contributing factor

- Planned invasive procedure with platelets <50,000/µL

• Isolated thrombocytopenia without bleeding or planned procedure does NOT mandate transfusion
• Elevated vWF partially compensates for low platelet count

Fibrinolysis

• Common intraoperatively and early post-transplant due to massive tissue injury, ischemia-reperfusion, and endothelial activation causing tPA release
• Presents as diffuse, non-focal surgical oozing (not single-vessel bleeding)
• TEG findings: prolonged lysis time, elevated LY30 (clot breakdown within 30 minutes)
• Management: Tranexamic acid in selected cases — but use with caution; over-correction risks thrombosis, including hepatic artery thrombosis (HAT)

Thromboelastography (TEG)

• Provides a dynamic, functional assessment of the entire clotting process vs. INR (measures only ~1% of the cascade)
