• Not appropriate for: Diffuse/innumerable disease, vascular invasion, metastatic disease, or uncontrolled AFP
• Purpose: Functions as a *selection tool* — identifying tumors with biology compatible with transplant

Criteria for Successful Downstaging (ALL must be met)

• Tumor burden reduced to within T2 criteria
• No new lesions on follow-up imaging
• No macrovascular invasion
• No extrahepatic disease
• Acceptable AFP behavior (declining or controlled with therapy)

The Six-Month Observation Rule

• A mandatory 6-month observation period applies after the first exception request
• This rule is not bypassed by treatment response, even after successful downstaging
• Rationale: Time functions as a *biologic filter* — aggressive tumors will declare themselves through progression during this window
• Immediate transplant listing following response without observation is incorrect clinical practice

Treatment Failure Criteria (Delisting Triggers)

• Continued tumor progression despite appropriate therapy
• Development of new lesions
• Development of macrovascular invasion
• Development of extrahepatic metastasis
• Uncontrolled or rising AFP despite treatment
