Portal Hypertension — Definition & Classification
- Defined by an increased pressure gradient across the liver (portal vein pressure minus inferior vena cava pressure)
- Classified as pre-hepatic, intrahepatic, or post-hepatic; >90% of Western cases are sinusoidal due to cirrhosis
- Complications include: ascites, varices, splenomegaly, portosystemic shunting
HVPG Thresholds
- >5 mmHg → Portal hypertension
- ≥10 mmHg → Clinically Significant Portal Hypertension (CSPH); associated with varices, ascites, decompensation, and increased mortality
- ≥12 mmHg → Threshold at which variceal bleeding can occur
- *Note:* HVPG is reliable in sinusoidal (cirrhotic) PHT but not in pre-hepatic causes
Primary Prophylaxis
- Indicated for medium/large esophageal varices or small varices with high-risk features (red wale signs, decompensated cirrhosis)
- Two evidence-based options (either is acceptable):
- Non-selective beta-blockers (NSBBs): propranolol, nadolol, or carvedilol (preferred in compensated ACLD with CSPH)
- EVL: for patients with NSBB contraindications or intolerance
- Doing nothing is not acceptable once high-risk varices are identified
Non-Selective Beta-Blockers — Mechanism
- Beta-1 blockade: reduces cardiac output → decreases portal inflow
- Beta-2 blockade: splanchnic vasoconstriction → reduces portal collateral inflow
- Carvedilol provides greater portal pressure reduction and is increasingly preferred in compensated disease
Secondary Prophylaxis
- Monotherapy is inadequate after variceal hemorrhage
- Standard of care: NSBBs + EVL (combination therapy)
- Either agent alone is associated with higher rebleeding rates and worse outcomes

- Restrictive transfusion strategy: Multiple trials demonstrate improved outcomes, including survival, when targeting Hgb ~7 g/dL versus liberal transfusion
- Over-transfusion increases portosystemic pressure → worsens bleeding and rebleeding risk
- Indicated in Child-Pugh Class C (score 10–13) and Child-Pugh Class B with active bleeding at endoscopy
- Early portal decompression significantly reduces rebleeding and mortality vs. continued medical/endoscopic therapy alone
