Foundational Framework

• Liver transplantation in cancer is biology-driven and protocol-driven, not diagnosis-driven
• Patients are stratified into two conceptual buckets:

- Compatible with transplant: Favorable tumor biology, controlled disease, criteria met

- Contraindicated: Aggressive biology, uncontrolled or metastatic disease

MELD Score Limitations

• MELD predicts short-term mortality from liver failure, not tumor progression, waitlist dropout, or metastatic risk
• Oncology MELD exceptions exist because biology-driven risk is invisible to MELD score
• No MELD score overrides uncontrolled extrahepatic malignancy

Absolute Oncologic Contraindications

• Active, progressive, metastatic, or uncontrolled extrahepatic malignancy
• Macrovascular invasion (main portal vein or hepatic vein)
• Extrahepatic metastasis (for most indications)

3. Disease-Specific Eligibility Criteria

Hepatocellular Carcinoma (HCC)

• Acceptable if within or downstaged to Milan criteria
• Requires: No macrovascular invasion, no extrahepatic spread
• Contraindications: Main portal/hepatic vein invasion, extrahepatic metastasis, ruptured HCC, T1 lesions (intrahepatic cholangiocarcinoma variant)

Intrahepatic Cholangiocarcinoma (iCCA)

• Single lesion ≤ 3 cm, biopsy-proven, unresectable
• Tumor stability ≥ 6 months after locoregional or systemic therapy
• Stability = no growth, no new lesions, no extrahepatic spread
• Size > 3 cm, multifocal disease, or extrahepatic involvement → excludes eligibility

Neuroendocrine Tumors (NET)

• Must be gastroenteropancreatic NET with portal venous drainage
• Primary tumor completely resected
• No extrahepatic disease for > 6 months
• Ki-67 < 20% (G1/G2 only); negative functional imaging
• Ki-67 ≥ 20% → ineligible

Colorectal Liver Metastases (CRLM)

• Disease strictly confined to liver; primary tumor completely resected
• ≥ 12 months of disease stability (most restrictive criterion)
• CEA < 80 µg/L required; values above this threshold indicate unfavorable biology
• Assigned MELD exception of MELD minus 20 (reflecting higher oncologic risk)

Hepatic Epithelioid Hemangioendothelioma (HEH)

• Biopsy-proven, unresectable, low-grade vascular tumor
• No microvascular invasion on imaging or pathology
• Extrahepatic disease is NOT an absolute exclusion (unique among malignancies)
• Indolent biology drives candidacy over anatomic location

Hepatic Adenomas

• Not a routine indication; transplant only for high-risk unresectable cases
• High-risk settings include:

- Glycogen storage disease, β-catenin mutation, liver adenomatosis (> 10 lesions)

- Progression despite non-operative management

- Hemorrhage, rupture, or malignant transformation risk

• Adenoma alone is insufficient; requires unresectability + high-risk biology
