Dr. Brett Fortune serves as Chief Medical Officer (CMO) of the Miami Transplant Institute (MTI), a unique affiliation between Jackson Health System and UHealth – University of Miami Health System. In addition to his executive leadership role, he is an active member of the liver transplant team. Dr. Fortune is a nationally recognized transplant hepatologist with extensive experience in liver disease and transplant operations. Prior to joining Jackson, he served as Medical Director of Liver Transplantation at Montefiore Einstein and played a key role in building and leading premier liver transplant and hepatology programs. An elected Fellow of the American Association for the Study of Liver Diseases and the American Society of Transplantation, Dr. Fortune is committed to advancing clinical innovation, multidisciplinary care, and research in liver failure, cirrhosis, and transplantation.
Advances in patient selection and optimal timing have expanded the role of TIPS in managing complications of portal hypertension. This session will review evidence-based strategies for identifying appropriate candidates, improving outcomes, and integrating TIPS into contemporary hepatology and transplant care.
Transjugular Intrahepatic Portosystemic Shunt (TIPS) has become a cornerstone intervention for selected patients with portal hypertension and its complications. Appropriate timing of TIPS placement and careful patient selection are critical determinants of procedural success, long-term outcomes, and overall survival. In this expert-led session, Prof. Brett Fortune will examine current indications for TIPS, including refractory ascites, variceal hemorrhage, hepatic hydrothorax, and portal vein thrombosis. Attendees will review evolving evidence supporting pre-emptive and early TIPS strategies, understand contraindications and risk stratification tools, and learn how multidisciplinary assessment can optimize patient outcomes. The presentation will also discuss the impact of TIPS on transplant candidacy, quality of life, and management of advanced liver disease in contemporary practice.
